Support at Home
YDHS is an experienced and trusted provider of home care support as part of the Support at Home program.
The Federal Government's Support at Home program helps you live independently in your own home for as long as you can. From 1 November 2025 the Support at Home program replaced the Home Care Package program
The program provides a subsidy towards a package of care services and care management to meet your personal needs.
Once allocated to you, a package is available for as long as it meets your level of need or circumstances.
There are eight levels of Support at Home Packages:
Level 1 - Cleaning, welfare checks, meal preparation.
Level 2 - Transport, light domestic help, medication prompts.
Level 3 - Personal care, OT visits, meal support.
Level 4 - Showering assistance, mobility aids, allied health.
Level 5 - Dementia care, continence support, case coordination.
Level 6 - Daily personal care, wound care, behaviour support.
Level 7 - Complex nursing, respite, mobility equipment.
Level 8 - Overnight care, hoist transfers, intensive support.

The new Aged Care Act 2024 commenced in November 2025, and is a rights-based law designed to put older people at the centre of the aged care system. Each level of package attracts different amounts of funding, with all levels being regulated by the guidelines and quality standards set out in the Act.
To be eligible you need to be assessed by the Single Assessment Service; this process begins with registration or a referral to My Aged Care. This is usually arranged by a general practitioner, nurse, or other health service provider, although you or a family member can also request an assessment.
When a package becomes available you will be notified of your package level and assessed contribution amount by Services Australia. This can be provided to any approved service provider, such as YDHS to support a discussion about your care needs and your services.
