Our main entrance to YDHS and the Yarram Medical Centre is located at 85-91 Commercial Road Yarram.

Crossley House Hostel
The entrance to the Crossley House Hostel is located at 14 Nicol Street Yarram.

St Elmo’s Nursing Home
The entrance to the St Elmo’s Nursing Home is located at Devon Street in Yarram.

Urgent care
The entrance to our urgent care unit is located at Nicol Street in Yarram.

Edgar House
The entrance to Edgar House is located at 20 Nicol Street in Yarram.

Visiting YDHS

If you have a specific enquiry and are intending to visit us in-person, we’d strongly encourage you to either phone or email us beforehand so we can make sure we’re able to source any information to assist with your enquiry ahead of time.

Information Sharing Schemes

The Child Information Sharing Scheme and Family Violence Information Sharing Scheme enable the sharing of information between authorised organisations to assess and manage family violence risk, or to promote the wellbeing/safety of a child.

Any request for the release of information pertaining to the above schemes must be done so by an authorised Information Sharing Entity (ISE) and/or Risk Assessment Entity (RAE). The requesting authorised person/agency will need to be verified prior to the release of information under these schemes.

To request information under one of these schemes, please complete the Information Sharing Request form and email your request to ydhs@ydhs.com.au.
