Aged Care Services for Yarram and Surrounding Areas
Yarram and District Health Service operates two residential aged care facilities offering high-quality aged care services.
Both facilities are located at our Yarram campus, with immediate access to our acute care, medical centre and allied health services.
St Elmo’s is a 30-bed residential aged care facility, which includes dementia-specific capacity, and carer respite while our 30-bed facility, Crossley House, provides residential services as well as carer respite.
Accessing our aged care services
All individuals seeking access to either of our residences must complete an Aged Care Assessment (ACAS). Your local doctor can provide a referral for an ACAS assessment.
Prospective permanent residents are also required to complete a Combined Assets and Income Assessment. This is available from Centrelink or from YDHS.
Completion of our Application for Respite in Residential Care or Application for Permanent Admission to Residential Care will also be required. The forms can obtained by calling YDHS on 03 5182 0233.
Complaints and compliments
If you have a compliment or complaint regarding the aged care services at YDHS, we'd encourage you to contact us in the first instance.
There are however several other bodies you can make a complaint with including the Aged Care Quality and Safety Commission.
