Acute-subacute ward
YDHS hosts a care ward with multiple beds where you can receive both acute and subacute care.
Our experienced team of health professionals will provide you with high quality care, including liaising with neighbouring health services to provide specialist care if this is required (including arranging transfer).
Step down acute care
If you’re recovering from surgery, illness or a health condition and wish to be closer to home, YDHS offers a step-down acute care program.
This program is focussed on recovery, rehabilitation and regaining independence, thereby reducing the risk of relapse of re-admission.
Transitional care program
Ready to return home after a stay in hospital but need a little bit of extra support?
Our transitional care program has you covered and will provide you with the supports you need to allow you safely return home while supporting your continued recovery.
In-patient palliative care
We take a person-centred approach to providing palliative care that is focussed on comfort and quality of life.
Our dedicated palliative care space has been established to provide sensitive end-of-life care for individuals and their loved ones and family members.
