Home Care: The YDHS Home Care service aims to ensure your home is a safe, comfortable and healthy environment. Household tasks to be provided, their frequency and length of time are based on individual assessment. We work with you to share tasks and develop skills to improve or maintain your ability to manage everyday activities. Home care is a HACC support service provided by qualified personal care workers, not a cleaning service. Home support workers can assist you to complete essential domestic cleaning, shopping, bill payment and banking, meal preparation.
Personal Care: Our Personal Care workers work with you to manage those personal tasks of daily living that, due to frailty or disability, you are unable to undertake fully by yourself. We will work in partnership with you to progressively improve, maintain and monitor your independence and ability to live safely at home and participate in community activities. An assessment is completed by the Community Nurse or Occupational Therapist to ensure safety for you and the personal care worker prior to commencing services. Personal Care workers can provide assistance or supervision with a broad range of personal tasks. Showering and dressing, mobility, food preparation, developing independent living skills are just a few examples.
Respite Services: provide a break for carers from their usual caring role to enable them to participate in community, social and other activities. They also provide an opportunity for the person being cared for to have a break, or an outing without their usual carer.
Property Maintenance: services provide modifications and minor repairs to assist people to maintain a safe and healthy home environment. This service is available to ensure the health and safety of YDHS Home & Community Care clients who do not have access to other means for this type of assistance.
Meals on wheels: are one option you may choose when we talk about ways to help you eat well. Meals are available for community care clients and people who have been recently discharged home from hospital.
Personal Alarm: Personal Alert Victoria is a personal monitoring and emergency response service which provides a daily wellbeing check and allows you to contact family and friends in the case of an emergency. Information and assessments for personal alarms are conducted by YDHS Care Coordinators.
