Yarram and District Health Service operate two residential aged care facilities. Both are located at our Yarram campus, with immediate access to our acute hospital wards, medical centre and allied health services. Each facility offers high quality residential aged care. St Elmo’s Nursing Home is a 30 bed nursing home which includes dementia specific capacity and carer respite and our 30 bed hostel accommodation, Crossley House, provides low care residential services also including carer respite.
An Aged Care Assessment (ACAS) must be completed by all individuals requiring access to either of our residences. Your local doctor can refer you to this service. Prospective permanent residents are also required to complete a Combined Assets and Income Assessment, this is available from Centrelink or from YDHS. Completion of our Application for Respite in Residential Care or Application for Permanent Admission to Residential Care will also be required.
