Discharge Planning

Proper discharge planning is an important aspect of patient care at YDHS.

Patient discharge is ultimately authorised by a doctor, however there is a large team of health professionals involved in discharge planning. Nursing staff (both acute and community), physiotherapists, dieticians, social workers and speech therapists are often all involved in developing a discharge plan for each patient that is tailored to their specific needs and circumstances.

Involving a range of health professionals and disciplines in the discharge planning process allows a holistic approach to the patient’s overall health and wellbeing to be taken. This is critical in providing patients with the best chance of making a full recovery and minimises the likelihood of re-admission.
