Diabetes Education Clinics

At YDHS, we are committed to supporting those within our local community with diabetes, through our diabetes education clinics.

Diabetes education is about assisting people with diabetes, their families and carers gain the knowledge, skills, motivation and confidence to manage their condition.

Who can see our diabetes educator?

Our diabetes educator is available for any person with:

  • Type 1 Diabetes
  • Type 2 Diabetes
  • Pre-diabetes
  • Impaired glucose tolerance
  • Impaired fasting glycaemia
  • A high risk of diabetes
  • Friends, carers and/or partners of the above

What does the diabetes education service at YDHS offer?

Our diabetes education services at YDHS includes a range of supports including:

  • Diabetes self-management program
  • Ongoing education and support
  • Inpatient visits at YDHS
  • Telephone advice
  • Community group education

When should I see a diabetes educator?

Our diabetes education services are available to anyone that falls in to one of the categories outlined above, however we’ve found accessing these services to be most valuable when:

  • You have been recently diagnosed with diabetes or pre-diabetes
  • You have been diagnosed with diabetes and have not seen a diabetes educator for more than a year
  • You need further information about diabetes
  • You’re seeking a change in treatment and/or experiencing unexpected problems
  • You’re looking for a little extra support

Do I need an appointment?

Yes, access to our diabetes educator is by appointment, with appointments available three Mondays per month.

What are the costs involved?

YDHS charges a small fee for its diabetes education services. This is reviewed annually.
